Thank you for your interest in accessing the CSPH’s Map of Campus Gender & Sexuality Groups!
Below you will find the application form for access to the map. Please read through and answer questions carefully and completely. To ensure the safety of all those listed on the map, there will be a background check of each applicant.
Members of the CSPH can gain access to the map at a discounted rate. (Interested in becoming a member? Click here.) Please note: discounts are not retroactive. Individuals can only receive lowered rates for access to The Map if they have already applied and been approved for CSPH membership.
Individuals who are not current members of the CSPH should continue down the page and fill out the form labeled “Non-Member Application for Access to the Map.”
Important: To receive your discount, current members of the CSPH must be logged in before applying for access to the map. If you are a member, please log in and click here to be taken to the current member application.
The CSPH will contact you via email within one week of receipt to notify you of your application status. Should you be accepted, you will receive an email containing a password and a link to a page where you can complete the application process and pay. Access to the map expires in 1 year; we will send you a reminder a month before your access will expire.

Terms of Use:
By accessing the map, you agree not to harass any of the entities listed or misuse any of the contact information contained herein. If a group and/or individual listed on the map asks you to not contact them again, you must respect their request. Should you fail to comply, your access to the map may be in jeopardy and a staff member will contact you by email and/or phone to discuss the matter. Additionally, if you are a member of the CSPH, misuse of this tool will be considered a violation of our terms and your membership could be terminated.
Access is for one individual only. Should we find through our tracking system that you are sharing your access, your membership to The Map will be temporarily suspended until resolution is reached with a CSPH staff member. Failure to respond within 72 hours will freeze your access to The Map. If you are a member of the CSPH, it will also put a hold on your membership privileges.
The Map of Campus Gender and Sexuality Groups is property of the Center for Sexual Pleasure and Health. Copying or sharing the information contained herein is prohibited.
If you breach these terms, we reserve the right to revoke access without refund.
All information compiled in The Map was either in the public domain or was sent to us by the individuals and organizations listed. If you find your name/group information on the map and wish to be removed, please let us know.
Non-Member Application for Access to the Map
Attention:

- If you are a student filing for access, please be sure to have a scan of both your current valid student ID and a proof of enrollment form. These files must be in JPEG or PDF format.
